First, a minor correction: there is no such thing as a "redbook break". The red book only specifies that there be a 2-second silence at the beginning. It does not specify or require silence between tracks.
Indexes are part of the CD format itself, so they are typically handled by the CD-burning software rather than the DAW. AFAIK all CD-burning applications allow you to specify indexes before burning. CD-Architect is an excellent tool if you don't mind giving a hundred bucks to the evil Sony corp. Nero works well, too.
There are advantages to exporting the entire concert to one large wave file, the most significant being you can decide how each song transitions into the next. If the band had some technical difficulties that held up the flow, you can shorten the gap. If the front man flubbed a bad joke, you can save him the embarassment by editing that out, too. You can even cut and paste applause if you want, to assure a wild audience reaction to even the weakest tunes. If you export separate files and let the CD-burning software do its default thing, you'll get boring 2-second gaps that may or may not be appropriate.
IOW, edit the big file in SONAR until it has just the flow and continuity you want, then export the whole thing. Then specify the track markers in your CD-burning program.
All else is in doubt, so this is the truth I cling to.
