Rewrite safe mode file open.

Rewrite safe mode file open so we are no longer forced to answer yes or no to each plug, but show us a sheet of them with [] enable checkboxes by all the plugs so we can pick and choose when troubleshooting files that have problems in SONAR please.
 
When disabling Melodyne for a song that crashed upon load, Melodyne was last in a line of what must be 50 dialog question boxes. That's not how it should be.
