Help with Track Envelopes

I don't know if I turned something off by accident or not but my issue is:
 
I can record fader moves with the RED W on the track and move the fader accordingly.  I see the envelope data being recorded.  During playback my volume changes can be heard but the FADER DOES NOT MOVE as if the envelope wasn't present.  
 
This cause much confusion during mixing in that I am visually seeing no fader moves but I can hear the envelope effect of the track getting louder.
 
So, I am wondering if this is some type of bug or if I changed a setting unbeknownst to me?

Sounds like you have inadvertently entered offset mode. Press "O" to exit. You can tell if that is the case by a + beside the fader. Offset can be helpful for making overall changes on a track with envelopes but can be confusing if you aren't aware you are in that mode. Still catches me from time to time even now.
