I can think of a few ways. One would involve drawing or playing a pitch change CC  that would be the easiest.
Hold your pitch bender, but then you'll need a tuner to see how accurate you are,, that's another thread going right now.

It is possible to change what the PRV displays for MIDI note values. The Note Pane uses the .Note Names section of Master.ins. The Master.ins is a text file which may be viewed and edited in any text editor. The Drum Pane uses a drum map for alternate note names. But these only affect the display. What the MIDI note values play is up to the instrument.

Well, it depends on what instrument you are using.  If it's a VST that has a fine tuner that allows you to transpose the instrument a quarter tone up, you can add two instances of the same VST using the same patch and detune one of them a quarter tone.  One instrument would be used for your "normal" notes, the other instrument would be used for your quartertones.

Cakewalk 3.0, i386 33mHz 2MB RAM, two 20MB IDE hard drives, Win 3.11 over DOS 6.22, Pro Audio Studio 16 16-bit Soundblaster compatible sound card, MPU-401 ISA MIDI Card, Digitech GSP21, Hiren Roy sitar, cow in a can

The pitch wheel should be on your controller ( keyboard) .
but you can draw then manually in piano view. I think cc# 16 is course tuning. cc#17 is fine tuning

Bigfrogs idea was the second one I had right away.Tune a second instance 1/4 step out and use it to play the funky notes. Not sure how your planing on making music with it as it will certainly be out of tune,,but there's always something new to try.

Assuming you have loaded a synth (or using a hardware synth) that is set to pitch bend range of 2 semi tones, insert a pitch bend event with a value of 2048 immediately before the note you wish to shift.  The most accurate way to do this is with the event list.

This will cause a C note to be raised to halfway between C and Db .

Don't forget to place a pitch bend event with a value of 0 immediately after the note off
(position of this corresponding pitch bend = 0 event can be checked with the PRV)

The pitch wheel will bend all notes playing on the channel, so not good for poly.
MIDI itself does not define microtuning, but synths are free to assign arbitrary pitches to midi note numbers.
If you want poly, the thing is to find a synth that supports microtuning, alternate scales, etc, then use scook's technique to keep track of what notes the MIDI note numbers refer to.
Cakewalk's dimension pro, rapture and z3ta+2 are said to support alternate scales ("scala" (.scl) microtuning definition files).
