How to properly name a song which uses a lot of samples from another song?

Hey everyone. So I'm a bit confused right now... I am currently working on a song which uses a lot of elements from another already existing (quite old) song. I have sampled those elements myself. However, this already existing song also itself uses vocal samples of another third song (which is even older). I have also sampled the vocal elements from the even older third song myself (plus another sample which the original song never had in it) 
 
Because of this I am left with elements of both the original old song, and the actual samples which the original old song used from the even older song, which I sampled myself. Now a lot of my song does use some (actually most) of the first old song. In fact it might be more accurate to say that it's a remix. However I have also added in quite a few brand new elements myself, to make my newer song. So my question is quite simply, how do I credit the original artists in such a way that I don't get sued for copyright infringement or something like that? Do I put ORIGINAL ARTIST NAME - ORIGINAL SONG TITLE (MIX BY ALI PRICE). Or what do I put? What's the protocol for these kinds of things. Anyway, that's the conundrum. Thank you to anyone who can shed some light on the subject for me.
 
EDIT: Just to clarify, it really is more like a remix than anything else, but I myself have added a whole bunch of new elements and completely rearranged the song.

    Actually, I think I might have my answer already. It's really not different enough for me to want to use any other format than: ORIGINAL ARTIST NAME - ORIGINAL SONG TITLE (MY MIX)

    I also often see

    as well.
