Might be a plugin that was failed before and has now been successfully scanned with the new sandbox?

This would be my first guess.

I suppose there could be an argument for this, although I would rather see an updated category instead. Currently the scanner reports updated plug-ins as "new."

Fortunately the log is easier to generate with the new option in Preferences > File > VST Settings.
