As far as I'm aware, the 'Great' in
Great Britain refers to the 'largest' island of the British Isles. That is the island that is made up of England, Scotland and Wales. The smaller islands that make up the British Isles (and mainly
Eire - the island that is made up of Northern Ireland and the Republic Of Ireland) would be described, comparatively speaking, as
Lesser Britain. Although I've never heard them alluded to as such.
The complication in nomenclature extends to the naming of the United Kingdom, which for reasons of geography/politics does not refer to an island or islands as such. Hence the UK's full title is
(The) United Kingdom of Great Britain and Northern Ireland.
