    Since I use fader offset mode frequently and I understand some others have had an issue with forgetting or not noticing they were in this mode (can't say it hasn't happened to me before) could we get the faders to change to a yellow or orange color while in this mode?
   It would definitely stand out and would be easy to catch your attention.
 
   Just a simple idea. Though I don't know how hard it would be to implement.

Anything to make it obvious that Offset Mode has been selected would be helpful. It's too easy to activate it accidentally and not notice. Coloured faders would certainly stand out.
 
Also I would prefer it if Offset Mode could be applied per-track instead of for the whole project.

FWIW, Offset mode affects more than just track faders:

So there would/should need to be some way to affect all of the other widgets as well.   That said, the current method of adding a "+" is something, though not all that obvious at a glance.  If colorizing widgets isn't an option maybe a highlighted/bolded border or similar...

How about a big ole letter "O" in the middle of the fader and pan pot if you're in offset mode?
