Re No.5 , yes, but it will also happen at the artist level - the drummers changes from ride to hi hi-hat, or vice versa and the sax comes in, etc.
 
 

#4 is an important one for me.
My old band recorded a 4-minute song.  Then had a producer spend SIXTEEN HOURS editing every note of every instrument.  The drums and bass were perfectly locked beforehand, with a great feel.  The producer removed every ounce of humanity from the track, and was paid to do it!
15 years later, I still can't listen to that track.  And would never work with that guy again.

Yup, #5 is where my head is on Rock (not metal) songs of late. Or, at least I'm trying to keep my head there.
 
Wasn't it Robert Plant that said Zep never played the same part (be it verse, chorus or intro) the same way?
