I was thinking that the on screen keyboard could detect how hard and fast you pressed your mouse button, or how long you held it down

there should be an input for a breathalyzer.

seriously, I built a neat little breath controller to work with my laptops microphone with pure data... as long as it's a crank call kind of breathing you're good.. my computer started to overheat though if i blew too hard though.
