It seems like even when I untick the checkmark for "Enable Midi Out", when I insert a softsynth, it still enables it.
 
Anyone else experiencing this?

There is no way to permanently disable "Enable MIDI Output" without hacking the VST registry for each plug-in and the hacks will get wiped out if the VST Inventory get rebuilt. That said, I have never had the "Insert Soft Synth Option" dialog ignored when used for each synth added to a project. Disabling the "Insert Soft Synth Options" dialog will cause the X3 default handling of "Enable MIDI Output", IOW if a synth can send MIDI data the option will be set on.
