MIDI only receiving on channel 1

I've gone into the preferences and tried to select 16 instruments and choose GM, but when I hit "apply" it reverts to only channel 1.  I thought it might be the controller, but it does the same with the onscreen keyboards in both Garritan Instant Orchestra and EW Play. 
 
Second issue - when I try to switch instruments using Keyswitches in EW play, Sonar crashes.
