Well - the midi Microsoft GS WaveTable is in the strict sense no MIDI device at all. It is in fact an MIDI converter. It is present only to "convert" raw MIDI files to sound. It works, but only when you are using something like DirectX drivers (not sure about that driver, because I never tried to do something like that).
When using ASIO drivers that "MIDI" device is not found as far as I know. The only MIDI devices that are useful in sonar are physical devices like a keyboard (non-qwerty if you know what I mean), contollers, MIDI interfaces etc. If you do not see anything in the set-up, chances are big you do not have a physical device...
There are ways around this. It is cumbersome, but it works. Try to find a software MIDI keyboard emulator. This program let you use your qwerty keyboard to input MIDI notes. There are also virtual keyboards that you can click with your mouse. Most of this software can let you do both. As I said - it is only a stop-gap, because this programs cannot sense things your qwerty keyboard does not has (like velocity, aftertouch etc.).
I do not want to sound harsh, but I think it is a good idea to take some readings about what MIDI is, and how to use it. Just use Google, and I am sure you will find something. Also - carefully read the first pages of the user manual, explaining the basics of MIDI and MIDI devices..
