It would be cool if we could add effects to our Custom Plug-in layout by right clicking the effect in question in the insert plug-in menu like windows "send-to" except the plug-in layouts and ultimately the layout folder structure would be selectable as a destination.
 
So for instance you could send that new flanger you are using more and more from the "All effects" view to your neat and tidy custom layout -> "Flangers folder".
 
I find that more often than not the effect I want is not in my default custom layout so I have to select the all plugins layout to get to it. It would be great to send it to my layout right there and not have to use up precious creative time and remembering to sometime update my layout which is becoming stale.

I had known about this feature before now :)
Would make things that little bit easier instead of sorting through unrelated stuff to find what I want. Got to learn to open my eyes and actually look past the end of my nose, there it has been sitting all this time When I get a bit of free time I'm going to organize my plugins into Compressors, EQ's, Reverbs etc etc, much better.

Plug-in manager is a separate executable (.exe program) so I'm not sure how integrated it is with SONAR.

Not from plugin mgr, from the insert plugin menu that pops up when you right click the fx bin.

The problem is where would the "folder structure" reside. It can't exist in the plug-in folder itself, because there are limits on how you can organize the dlls - try creating folders and moving the plug-ins around, it doesn't work as you might hope. So it would need to work through an intermediary, like plug-in manager, or Cakewalk would have to re-invent how the VST plug-ins folder works (and there's also DirectX to consider, as well as the different way VST3 is spec'ed to organize plug-ins).

I get what the original post asks for, but actually I find the way it currently works to be tidy and fast.  I like how it works presently, being able to pick and choose and decide where you want to place the various things in the Plugin Manager.

I'm pretty sure that the custom user effects layouts don't contain actual dlls but references to them instead.
They can be thought of as simply filters of the master big list.

I am not sitting by my studio pc right now ,but I have used the plugin manager for quite some time to organize plugs in different folders. Either by category, or by producer. You have to save the preset (with any name you choose). When you right - click in the effects bin, on top, there should be (I think) something called preset manager. Follow that, and click on your saved preset. I don`t think it contains any dll`s, only references to which plugins you have selected.  Works like a charm for me.
