What I think might be useful is some obvious indicator making it clear when offset mode is engaged. This could be a button on the Control Bar. When first activated let it flash then goto a steady bright color.

I have NEVER used offset mode intentionally. I have no idea how it got set. I think a setting to turn it permanently off would be a better idea. :)

It is possible to disable the keyboard shortcut which invokes offset mode. Bind "O" to "Do nothing" in Preferences > Customization > Keyboard Shortcuts.

IMHO, the 'o' key should do nothing by default.
 
How many times have we helped troubleshoot for frustrated users due to the offset button?
 
Most people who would intentionally use off-set also know how to do key binding.

I like the do nothing approach. I have reset the 'O' to do nothing. I will NOT be bitten by this again I hope.
