+1 on making sure 'Local' is turned off for the external synth, when you are working with it and Sonar, or it will generate local sound.

Turn off playback so you'll only hear from the midi module, not from your controller keyboard:
Preference-> MIDI->Playback and Recording. Under Playback, Un-check Always Echo Current MIDI track
