I was gonna say ... a nice psychedelic pacifier ... but maybe not.
 
I'd just give the mom a nice Gift Certificate, and let her make the call. Toys are kinda fun for 2 hours and then done! We do the same thing here with songs!

A book.  The baby may get (hopefully) a few of them, but the likelihood of getting
the same ones isn't that big.

We all came (9 children!) from a house of literature with over 40K books of literature. I can honestly tell you that if someone/anyone gives me another book, they will get a picture of what to do with it a week later! 
 
That is an area that is so mis-represented and understood, it's not funny. Sort of like ... someone giving me a Harry Potter book, and I am not into fake mysticism, or cardboard/movieset mysticism. Now, if you give me a nice hardbound of Whitley or some fancy name out there in the world of the real arts, instead of mumbo-jumbo, I think that not only will I read it ... I will enjoy it!

Isn't 4 months when most kids are getting smartphones and iPads?  Why the question, does this kid already an iPhone/iPad combo?

I think this baby gift boat sailed months ago folks. The kid's probably paying taxes by now.
