Don't feel bad. I have 2 novels working and a third one on hiatus. The two stories were just happening, and I can still see them in my head, but sitting down to write them? ... I probably need to take a week off work and be tied to a chair!

I blame the author, Karyn.  I've been told that he's something of an Idjit.  He's not going to be entrusted with such responsibilities in the future.  When the next book is finished, he'll humbly ask for volunteers to proofread it before it is inflicted on an unsuspecting public.  A few of my Coffeehouse friends have been very helpful in pointing out some brain glitches.  
 
Also, I share your daughter's disappointment.  My printed copy isn't signed either.

Perhaps you could use your time machine to go back and sign her copy?
