1)  She's the NEW triple-breasted prostitute for the re-do of Total Recall
 
2)  It's not PhotoShopped, but they are NOT real (just good costume effects)
 
3)  The ones on the actress in the original Total Recall (with Arnie) weren't real either.  That said, it's pretty rare, but some girls are born with three breasts, though most have the middle one removed to look more normal.  Note that it can happen to guys too!  I knew one guy when I was young in San Diego that had three; two in the normal positions and one in the middle that was a little higher and closer to one side.
