Here is a first attempt at converting the audio clip to MIDI with Sonar, then playing the file back on a MIDI instrument.  Used a lead guitar for the first voice part and then acoustic piano for the whistled part.  Hope this is close?

The pitch detection was initially thrown off by frequencies outside of the main vocal phrases, so I used the LP-MB compressor to sharpen the focus on the strongest frequency bands in the melody.  Then the conversion seemed to work fine.  The quality of the microphone and the clarity of the signal would seem to be the biggest factor in getting a good voice to MIDI conversion.  Maybe the kazoo is not such a bad idea?
 
Had to split the MIDI file into two tracks, one for the first part, and another for the second.  Then used the MIDI transpose process to change octaves to those that matched the instruments for MIDI playback.  The only effects used for playback were already in the instrument programs.  All processing was in the box.

Thanks much for the effort. It seems I still have the same issue tho - the pitch of many notes is off and requires me to manually transpose them. And notes are missing. So going from audio to MIDI using software seems difficult.  

I guess what I need is someone to actually play a MIDI device based on my audio and record it, with velocity and pitch bend, etc. It takes me over a month to do what someone else could do in half an hour. I may put up an official Help Wanted post.
 
In the meantime, I will likely try the kazoo and maybe a MIDI device to play monotone notes and transpose 1 note at a time.

Good luck with your project!  Hope you find a workable solution.  From what I have seen, the single track audio to MIDI conversion gets confused by any harmonics or pitches, including breath noise, outside of the range of the intended musical phrase.  So the closer to a pure tone that you can produce, would be ideal for the software recognition.  I have heard that Melodyne offers the latest and greatest in this field.  But it's still a challenge ...

I split this into two MIDI tracks, channel 1 for the first part and channel 2 for the second, because the voice and the whistle were octaves apart.  Only transposed them by octaves to get the MIDI instruments to play the parts correctly, but no editing of pitch or note duration was attempted.

I'm unable to play an instrument or use a MIDI device, so in order to get a melody into Sonar I have to either use my mouse and click each note in (a pain), or find a good audio to MIDI program. Melodyne is not doing a great job, whether i hum as clearly as I can, or whistle. Just the melody.
 
Is there any other audio to MIDI software that do a good job?

The difference in in pre-processing the audio. I split it into several chunks and have tried different effects.
But the strategy was the same: level the signal up to some reasonable dB (I have clearly "overdone" it for the second phase), hard cut everything outside the "zone of interest", I mean <120Hz and >5kHz, by gating and compression eliminate as much "wrong" sounds (at the beginning and at the end of the notes) as possible.
