Offset mode used to be hotkeyed to "O" but was hit by mistake and caused enough grief-stricken posts that it was removed from the "O" assignment.

The small "+" is there but only when offset mode is active I think. I totally support the idea of having a visual indication that SOME offset is taking place though!
 
As an unrelated question that just occurred to me (and sorry for going OT), is offset mode absolute? In other words, if I use it to boost by 6dB a track with some automation on it, does it simply add 6dB everywhere or does it somehow intelligently take into account the intended automation?

Automation will "ride on top" of whatever Offset you have entered. That's why the "normal" state is called Envelope mode.

Right so if I have automation that fades to zero in places, I can't use offset mode, basically?

I did check. I saw no offsets at all. I have no reason to lie. And I didn't have offset mode 'on', either. I made this from the blank template and hadn't used it. there was never anything on any track til I added that clip to test it. So how did offset mode come into play? What do I need to learn to catch this?

Just remove the offset from the Gain and Drum master bus and then save the template in the template folder with the same name. Open it again see if it ok now once the changes are completed

Just checked my Template and its perfect so you must have accidentally hit the wrong key somewhere. No harm done and the guys on here were quick to help and sort out the problem

I didn't mean to suggest you "lied" as much as that apparently you checked in the wrong place or there was some sort of miscommunication or oversight. I was just amused that in fact the "solution" that was suggested first turned out to be the right one, and we got so side tracked :)
No offense meant, I learned from the whole metering discussion though, it's all good.

Maybe somehow your blank template was modified and saved? You're definitely not the first person to be tripped up by offset mode. Kind of a mixed blessing.

Technically, that's correct for positive offsets, but it could be argued that you'll never hear the difference between -Inf and -Inf +6dB.

Possible, but that depends on what's going on elsewhere and how loud the clip is, I guess. Also, for a negative offset, my fade points would move. Good to know, I quietly assumed there would be some intelligent curve handling.

I still don't see any 'offset'. What am I looking for, and why would it work if I don't have offset ticked in the control bar?

The Offset icon in the Control Bar is how you enable/disable Offset mode, but it's the value of the widgets in either mode that matters.   Check for differences in the respective faders with Offset mode disabled and enabled - in Offset mode they'll have a small plus sign next to them - that should point you in the right direction.
